Self-help program

Anxiety questionnaire

You filled in an anxiety questionnaire at the start of the self-help program. You can now fill in the questionnaire again and see whether your anxiety has decreased during the self-help program. You can also come back and complete the questionnaire later on after you’ve done more exercises.

Anxiety screening (GAD-7)

This questionnaire will help you assess the severity of your anxiety symptoms. The questionnaire is based on the DSM-5 diagnostic criteria for Generalised Anxiety Disorder. You can answer the questionnaire regardless of whether you have a diagnosis of an anxiety disorder.

Instructions

Carefully read below for instructions on how to answer the questionnaire and how to calculate your points.

You can answer the survey anonymously. Your answers will not be saved.

Please note

If you want a health professional to assess your situation, please contact the healthcare provider responsible for your care.

Have your symptoms decreased?

Great! There’s no reason to worry if your symptoms haven’t completely disappeared but just decreased. The most important thing is how they affect your life. If you believe that you can live a fuller life than before, you’ve already achieved a lot. Recovery can also continue after completing the self-help program.

If your symptoms have not decreased

If it feels like you still need more support, tell a professional about this. You’ll find tips for this a little later.

Many things in everyday life have an impact on improving your well-being. Now, we'll look at a few things that affect your general well-being.

Remember
